Initial Assessment

This is a quick, baseline assessment to gauge where your stress levels are — in your brain, your body, and your life. Your answers will help me better understand how stress is impacting you and where support is most needed. Because of that, please answer as honestly as possible. Your responses are completely confidential and will not be shared with anyone. If we decide to work together, we’ll do a more thorough assessment before our first session.
